一、依據高級中等以下教育階段非學校型態實驗教育條例（以下稱本條例）第 6 條第 1 項規定：「前條申請人應填具申請書，並檢附實驗教育計畫，至遲於每年四月三十日或十月三十一日前提出申請。」
二、依上開規定，本市申請案採線上申請，資料上傳時間訂於 114 年 4 月 1 日（星期二）至 4 月 30 日（星期三）止，請申請人完成線上申請後，於 114 年 5 月 5 日下午 4 時前，將完整申請書紙本送至申請人學區設籍學校（如附件二、三），未送交或申請資料不全者，不予受理申請。
三、有關設籍學校之規定，說明如下：
(一)依本條例第 15 條第 1 項規定：「參與國民教育階段個人實驗教育之學生，其學籍設於原學區學校；參與團體實驗教育或機構實驗教育之學生，其學籍設於受理辦理實驗教育申請之直轄市、縣（市）主管機關指定之學校。」
(二)次依教育部 112 年 5 月 5 日臺教授國字第 1120058502 號函釋：
１、個人實驗教育之學生學籍設於原學區學校，倘私立學校
定設籍學校時，應以公立學校優先作為團體及機構實驗教育之設籍學校。
３、綜上，個人、團體、機構之設籍學校應以本市所屬公立學校為主。
四、線上申請網址為： https://nee.tn.edu.tw/，由申請人註冊填寫。
五、國小六年級申請國中階段，由未來設籍國中學校辦理；國中三年級申請高中階段，上傳資料由承辦學校線上審查或由未來欲簽訂合作計畫高中辦理。請申請人在申請頁面中，務必選擇「國中」或「高中」，再輸入申請資料，若選錯學制，需回到上一步驟，重新輸入個人資料。
六、為使申請人熟稔申請及賡續辦理程序，可參考申請流程影片 https://youtu.be/KE4tMSheX9w ，請各校公告於學校網站，並轉知欲申請人參考使用。
七、申請人所繳交之正本申請書，由設籍學校留存，不必再寄至承辦學校隆田國小。
八、請各校留意作業期間及截止收件日期，以維護申請人權益。
