說明：
一、 近來常有特殊教育學生家長因家庭因素或其他因素，在未知會原就讀學校輔導室 (或特教組)，逕行透過教務處(註冊組)，以一般學生身分轉學，致新轉學學校於不知學生為特殊教育生，透過學籍編班系統，進入普通班，造成新安置學校之班級導師，無法事先取得學生身心特質與輔導因應方式，易造成個案班級適應與級任老師經營班級之困境。
二、 為避免上述情事發生，請學校務必加強宣導，身障學生導師於接受家長轉學訊息時，除告知家長特教學生轉學需透過教育局(鑑輔會)安置程序轉學，不宜直接到新學校報到並知會輔導室 (或特教組)有轉學情事。避免新就讀學校未透過轉銜會議，無從了解學生身心特質，造成不知該如何介入輔導安置班別，亦對學生未來環境適應甚為不利。
三、 若經溝通告知後，家長(個案監護人)仍私下以一般生到新就讀學校報到，請新就讀學校註冊組(負責轉學生編班行政人員)，無論個案是否為特殊教育學生，都應先與個案原就讀學校聯繫，確認個案是否有學習上之特殊需求，以避免家長隱瞞學生特教身分，造成新安置學校未事先因應，以一般生編班程序安置特教學生，造成事後導師輔導、班級經營與學校特教行政之困頓。
四、 綜上所述，請貴校加強宣導校內特教學生轉安置運作程序及各處室之間橫向聯繫與溝通，以減輕校際之間學生安置輔導事務。
